simplify melody send/return

I no longer want to use two mono sockets for separate send and return
routing. A single stereo jack for effects insert is just as fine and
easier to implement. I also no longer care for an effects blend
potentiometer that determines how much of the unprocessed signal is to
be blended with the return signal. Controlling the effect intensity is
the external device's job.
